Ischemic stroke occurs when a region of the brain has decreased or absent blood flow that causes ischemia and eventual infarction. Approximately one third of infarcts are caused by emboli and the remaining two thirds are caused by thrombosis of a vessel. The most important cause of infarct is atherosclerotic vascular disease that can lead to thrombus or emboli. Another main source of emboli is the heart (atrial fibrillation). Less common, infarcts are caused by global decreased perfusion, vasculitis, coagulopathy, or vasospasm (after subarachnoid hemorrhage).
